Low Atrial Rhythm vs. Junctional Rhythm: The PR Interval Holds the Clue

A low ectopic atrial rhythm and a junctional rhythm with retrograde atrial activation can appear remarkably similar on the ECG. In both cases, the P waves are often inverted in the inferior leads (II, III, and aVF) because atrial depolarization travels superiorly rather than inferiorly. The key distinction lies in the PR interval.

In a low atrial rhythm, the impulse originates within the inferior atrial myocardium, just above the AV node. Although atrial activation is directed upward, the impulse must still travel through the AV node before reaching the His Purkinje system and ventricles. This normal AV nodal delay produces a PR interval of at least 120 ms.

By contrast, in a junctional rhythm with a retrograde P wave preceding the QRS complex, the impulse originates within or near the AV junction. Retrograde atrial activation begins almost immediately if it arises from the more superior portion of the AV junction, while a second wavefront travels into the ventricles. Because the atria are activated from a location already within the AV junction, the interval between the retrograde P wave and the QRS is very short, typically less than 120 ms.

Understanding the underlying anatomy makes interpretation straightforward. An inverted P wave with a PR interval ≥120 ms favors a low atrial rhythm, whereas an inverted P wave with a PR interval <120 ms strongly suggests a junctional rhythm with retrograde atrial activation. Rather than memorizing patterns, visualize where the impulse begins and the conduction pathways it must traverse.
